Northern Michigan Diabetes Prevention Coalition
The Northwest Michigan Chronic Disease Prevention Coalition in collaboration with the Northern Michigan Chronic Disease Coordinating Network and the Northern Michigan Diabetes Initiative have formed the Northern Michigan Diabetes Prevention Collaborative. The purpose of this collaborative is to ensure coordination between Diabetes Prevention Program (DPP) providers to achieve the best outcomes for our region.
The collaborative will accomplish this through the following:

DPP Action Plan

Provide technical assistance

Coordinate DPP scheduling to ensure even regional coverage

Share lifestyle coach tips for success and best practices

Communicate through meetings, emails, and other avenues as appropriate
